Apply for & Manage Grants

Lists all grant resources for grantees to apply, manage, report, and close out demonstration grants and waivers.

This page provides useful resources when applying for, managing, and closing out a Section 1115 grant.

When drafting a grant application, the best source of information is the grant’s notice of funding opportunity (NOFO). The NOFO will provide specific details about eligibility, disqualification factors, required federal forms, content and formatting requirements, submission instructions, review criteria, and frequently asked questions.

  • Finding and Applying for Grants - provides information about applying for a grant, including finding and understanding funding opportunities, submitting an application, understanding the review process, and more.
  • Grants 101 - provides information on the grant lifecycle, including pre-award, award, and post-award phases. It also includes a checklist for getting started.
  • HHS Grants Process - describes the process for ensuring proper planning of grants, fair competition, applicant assistance, and grants management.
  • ACF Evaluation Framework - describes the different types of research and evaluation and outlines their roles.
  • How to Create a Logic Model - provides key steps for creating a logic model.

Most forms are maintained on Grants.gov. The Forms Item Description page for each form has the downloadable form, instructions, and other detailed information. Contact your grants management or project officer for information on how and where to submit the form.
