Introduction
Sustaining Tribal TANF-Child Welfare (TTCW) collaboration and service coordination rests on having a clear plan. This Sustainability Planning Workbook provides information and worksheets to help TTCW grantees and partners develop a feasible plan for program sustainability. This workbook includes background information about sustainability planning and two distinct sections of worksheets. The first section focuses on documenting your program model, with worksheets to help you identify and elaborate on your program goals, outline the program components to sustain, assess ongoing staffing needs, and explore available partners and resources. The second section will guide you in creating your sustainability action plan; it includes two worksheets to reflect on your program strengths, opportunities, challenges, and barriers, and to finalize your sustainability action plan. The Appendix includes links to additional publicly available resources that cover sustainability.
Purpose
This Sustainability Planning Workbook is intended to be a resource for Tribal TANF-Child Welfare Coordination grantees and other partnering human services programs to support program sustainability planning.
